Another point, they often threw an answer choice saying "Nothing else to be done, Reassurance, Normal for age" which I really found annoying! The answer choices are often a path-phys description of the disease, and asking for the 2nd line diagnostic test/ treatment option, just be prepared for that. I kept telling myself that if the usmle decided to baldly shorten the exam for some by 80 qs while thinking this is fair, maybe that means this last block was experimental (given how hard and vague the qs were)? I know this is a long shot but that was how tired I felt at that point. I had 2 abstracts, the first one was OK, the 2nd one was the last thing I did on the last block! At that stage I was drained, I couldn’t even think or read the abstract or the qs. Surgery qs were scarce, biostats straight forward (mostly easy calculations: sens, spec, PPV.) and maybe 4 ethics qs. Those qs were on the harder side actually. Also I had a lot of peds on my exam, mostly covered by uworld. The questions were vague, I tried my best to apply the criteria but that was hard, sometimes two diagnoses fit the case, I was really pissed off! I had a q about almost every major disease. Know everything (screening, diagnosis, prognosis.) about the major chronic diseases (HTN, DM, Hyperlipidemia, etc) in kids and adults. Those qs were ones of the hardest because they were so personalized and there was no way to prepare fully for them. I had a fair number of "most important risk/prognostic factor" qs. I had 3 auscultations (very easy, the stems have all the information you need to answer the qs without even listening to the murmur) and about 10 ECGs (mostly easy with some weird ones). The middle ones were more reasonable, and I was flagging 6-12 qs/block. My first and last blocks were crazy!, I flagged about 20 qs/block. It was HARD (as you already know), but doable. I took the self assessments in the last 2 weeks before my exam in that order: Other than that, I listened to some of divine's podcasts and used google search to read more about some topics. At the end, I had a book made of about 80% of the information in uworld! And based on that I can say that about 20% of my exam topics were NOT covered by uworld. I did the qs subject-wise, tutor mode, and whenever I finish a section I would go back to the explanations and study them while taking notes and adding them to the uworld hand outs.
